Job Description

Our client is seeking strategic, hands-on leaders to join the Board of Directors for a healthcare services platform. This is an equity-based opportunity with quarterly in-person meetings and ongoing strategic engagement expected.


CLIENT:


This founder-led healthcare organization provides facility-based services. The organization is poised for growth and transformation with the support of a private equity partner.


ROLE:


The Board of Directors will serve as strategic partners to the executive team, supporting operational scaling, financial growth, and value-based care initiatives. This is an opportunity to join a high-impact board at the ground level and contribute meaningfully to both tactical and strategic decisions. Board members are expected to be highly engaged offering guidance, introductions, and insights that accelerate value creation.


The ideal candidate brings healthcare experience, a growth mindset, and a willingness to roll up their sleeves. Previous board experience is welcomed but not required.


Responsibilities:

Governance & Oversight

  • Provide strategic input on company initiatives, growth opportunities, and operational optimization

  • Review and advise on quarterly financial performance, KPIs, and management reports

  • Participate in board meetings (3x/year in Chicago, 1x/year in Nashville or Naples) and investor calls

  • Collaborate with the CEO and PE sponsor to support business milestones and long-term objectives

Strategic Engagement

  • Offer introductions to potential partners, customers, or add-on targets

  • Guide management in navigating facility-based care operations, compliance, or risk-bearing models

  • Support recruitment of key executives, such as CFO, VP of Finance, or Growth Lead

  • Actively contribute 6+ hours per quarter beyond board meetings


CANDIDATE:

Qualifications:
We are seeking board members with deep experience in one or more of the following areas:


Senior Living / Facility-Based Care Operators

  • Former CEO or COO of a scaled chain in assisted or independent living

  • Expertise in operational execution and cross-state growth strategies

  • Strong industry network and firsthand experience scaling facility-based businesses

Value-Based Care Experts

  • Former leaders at organizations like Oak Street Health, VillageMD, or similar

  • Deep understanding of geriatric care models and risk-bearing payment structures

  • Experience delivering care in value-based environments

Payer-Side Executives

  • Former executives at payers such as UnitedHealth, Humana, or similar

  • Skilled in risk-based contracting and insurance product design for senior populations

  • Strong compliance and policy orientation

Other Considerations

  • MDs with operational or business acumen

  • First-time and seasoned board members welcome

  • Prior PE board experience preferred but not required

  • Limited current board seats (ideally fewer than 5 concurrent boards)

Compensation:

Equity-only structure with co-invest opportunity and projected 3x capital return. No annual cash retainer or travel reimbursement. Opportunity for additional advisory or sourcing fees.
